Incident reporting
Report ALL injuries, illnesses, near-misses, property damage and dangerous occurrences. Same day where possible.
What to report
If anything goes wrong — even if no one was hurt — log it. Near-misses are how we find weaknesses before someone gets hurt.

If something happens

Make the area safe
Stop work if needed. First aid for any injury. Isolate any failed equipment. Don't move things unless you have to — preserve the scene if it might be a notifiable event.

Notify
Phone the workshop owner immediately for anything beyond a minor scratch: Sam Gibbons on 021 926 299. For a notifiable event, also call WorkSafe NZ on 0800 030 040.

Submit a report
Same day where possible. Use the form below — it sends a PDF straight to the workshop owner with everything WorkSafe expects to see.

Investigation (within 7 days)
For anything more than a minor incident, the workshop owner runs a short investigation: direct cause, system cause, corrective actions. The report you submit is the starting input.

Submissions are emailed to Sam Gibbons (the workshop
owner) and a PDF is attached for filing in the Incident Register. Source
